How to Apply for Residency
At Ingersoll Place, we work closely with prospective new residents and their families to provide insight into the decision-making process and make applying for residency as easy as possible.
The best way to begin is to give us a call at (518) 370-4419 or contact us via email. We can answer any questions you may have about our facility, services and fees and will schedule a tour for the prospective resident and/or family members and caregivers who may be involved in the process.
Once you’ve made the decision to move forward at Ingersoll Place, this brief outline may be helpful in understanding the admission process and what steps need to be taken:
- Complete the Ingersoll Place Application Form. It can be faxed or emailed in advance or hand-delivered on the day of the pre-admission screening. (See next bullet.)
- Schedule a pre-admission screening with our Case Manager or Resident Care Director. The screening takes approximately one hour and allows Ingersoll’s staff to assess the prospective resident’s needs and ensure that Ingersoll Place is well-suited to meet them.
- Upon completion of the screening and once preliminary approval for residency has been given by Ingersoll’s Resident Care Director, the prospective resident and/or family will need to schedule a physical exam and Tuberculin Skin Test (PPD) with their primary care physician.
- To ensure the apartment of choice will be available at the time of move-in, a $1,000 deposit should be made at this time. The deposit will be applied to the first month’s rent or, refunded in full – for any reason – prior to the move-in
- As required by New York State Department of Health, Medical Evaluation forms must be completed by the primary care physician along with written orders for medications and other services such as physical therapy or visiting nurses. These forms are returned to Ingersoll’s Resident Care Director for final review prior to admission to Ingersoll Place. Please note: Typically, a move-in date is scheduled within 30 days of the pre-admission screening. If there are delays that prevent move-in within that period of time, a follow-up screening with Ingersoll staff will be scheduled to ensure compliance with Health Department guidelines.
